Community Health Worker Network Mission: 

The mission of the  Community Health Worker (CHW) network is to connect CHWs to resources and services in the district to help CHW better serve their communities.

Community Health Worker Network Goal: 

The goal of the CHW network is to connect health care providers and services to current CHWs. CHWs role in the South Heartland District is crucial for equal access to care for all communities members. The network will help supply CHWs with tools, education and resources for their communities they serve. The network will also serve as a learning opportunity for CHWs to enhance their skills to better serve their current and future communities.

Liz Chamberlain

Area of Expertise:

Liz has lived in the Hastings community since the late 1980’s. She has spent the last 25+ years working with people of all ages including veterans referring them to the services they need in the community. As South Heartland District Health Department’s Fall Prevention Coordinator she educates older adults  in the steps to take to prevent falls.

Fun Fact:

Liz is actively engaged in the Boy Scouts of America and enjoys shooting archery in her spare time.

Lorena Najera

Area of Expertise:

Lorena has lived in the Hastings community for over 25 years. She has served in a community health worker  (CHW) role connecting people in her community to resources for a total of 18 years. Lorena joined the South Heartland District Health Department team in 2015 after having worked at Head Start and DHHS. As South Heartland’s bilingual CHW, she works each day to connect the Hispanic community with resources to benefit themselves and their families. Lorena is passionate about helping others to live a happier, healthier life.

 

Fun Fact:

Lorena had a 6th grade education when she moved to Nebraska. When her children were young, a CHW from Head Start, paid them a home visit. This CHW was the key to her engagement in the American culture and interest in earning her GED and post high-school degree. Lorena’s experience with this CHW from Head Start helped bring her to where she is today, serving others in her community as a CHW.
