The Public Health Outreach Nursing Education Nurse (P.H.O.N.E.)
Program. administered by South Heartland District Health Department is
working with the Nebraska Department of Health and Human Services to
provide outreach and education to clients receiving or potentially
eligible to receive Medicaid or Kids Connection Health Care Coverage.
The Registered Nurse provides supportive health information to families
to assist with finding health care.
Primary Goals include:
Assisting families in finding a primary care provider, either medical, dental or vision.
Encouraging parents to make sure their children receive their well child exams
Helping families to identify and overcome barriers to health care and ensure appropriate access to care.
Providing medical follow-up and coordination to assure client
compliance with medical treatment as requested by the Medicaid provider.
Assisting in the reduction of occupancies of failed appointments through counseling to assure receipt of care.
